Join us for a conversation about the most common misconceptions about hospice care. Find out who qualifies for hospice, the best time to enter hospice and what hospice care consists of for patients dealing with chronic disease. This presentation can help you make better care decisions for yourself or loved one. An interdisciplinary team from JSSA Hospice will co-lead this presentation.    

MariaKaplan, RN, BSN, CHPN, a certified Hospice and Palliative Nurse, will address hospice philosophy, patient eligibility criteria, the referral process, scope of services, insurance coverage, and more.

Bridget Burke, LMSW, APHSW-C, a hospice social worker and bereavement counselor, will address emotional, social, and cultural support available for hospice patients and their families.
